My Diabetes Home is a user-centered technology platform that helps people with diabetes make informed health choices, and collect medical information for assist their providers. Working closely with the medical doctor who founded the company, we created an identity that personified the three distinct elements of My Diabetes Home: the individual, the concept of the medical home, and idea of taking ownership of personal well-being. The mark feels warm and inviting and brings a much needed human element to what can be a serious medical issue.
